This very hip 70 something retired teacher had implants so old (41 years) that I thought of calling the breast implant museum! These implants were rock hard because the scar tissue around the implants, which were ruptured, was calcified. I removed the ruptured implants and the scar tissue and gave her a set of the newest generation of implants. She and her husband are thrilled with her nice result. Many ladies in this age group think they are too old for surgery. If they are in good health, age doesn’t matter very much.

This patient had 25 year old gel implants on top of her muscle. They had started to ripple. I removed these 360 cc implants and replaced them with 250 cc implants under the muscle. I also added some Seri surgical mesh to the lower part of the breast to add some thickness and also help control the implant position. She is several years out from surgery and is doing very well.

This 40 year old lady had 6 year old saline implants that looked terrific but she felt they were adversely affecting her health. I do not think breast implants cause systemic illness but I am happy to remove implants from anyone for any reason. It’s the patient’s choice. I removed her implants, which were normal and also removed the scar tissue around the implants. The scar tissue was quite tight. She has had some improvement in her health and has had a lot of relief for some chest tightness that was likely due to the scar tissue.

This 58-year-old personal shopper had five implant procedures over 20 years (including removal, replacement and revision of gel implants) by various surgeons. Three years ago she presented with a left implant that rode too high and a right breast with a “socked in” scar. Her left implant pocket was lowered and her deformity on the right was found to be a malpositioned pectoralis muscle. The muscle was put back into position on her rib cage. New gel implants were placed above the muscle and so far, so good. Maybe the sixth time was a charm.

Before her augmentation, she had significant breast asymmetry and she knew that one implant was larger than the other. She presented to me with deflation on the left side. We were unable to obtain her records from her previous surgery which would have indicated the sizes of the implants. I elected to purposefully deflate the right side so I could examine her and size her for new implants. I removed her deflated implants and re-augmented her with Silicone gel implants 300cc on the left and 371cc on the right. We were both very pleased with her final result.

This 38-year-old mother of two developed a fluid collection (seroma) around her left breast implant. This is a rare complication of breast augmentation. I have had two such patients in my practice since 1991. After drainage, the seroma recurred and she elected to have her implants removed. She is thinking of having another augmentation.
