Buttock Enhancement a.k.a. Brazillian Butt Lift

Buttock enhancement is done for women (or men) who feel their backside is too small or too flat or too saggy.  Yes, even in Seattle, this area gets a lot more attention that it used to and buttock enhancement is a procedure that more and more patients are asking about. Many people refer to this procedure as a Brazilian Butt Lift which sounds a bit coarse to me, but maybe that’s because I’m from Spokane.

Quick facts:

  • Costs: $7200 – $17,000 depending on the type and length of surgery and number of donor sites.   Fat transfer from the hips to the buttocks costs a lot less than a buttock lift combined with fat transfer from the hips to the buttocks.
  • Post-operative pain: quite a bit. 
  • Time in support garment:  3 weeks.
  • Time off work or school: 1 – 3 weeks depending on the type of work. 
  • Time off driving the kids to lacrosse:  1 –  2  weeks or once you are off narcotic pain meds.
  • Time off the gym:  3 weeks.
  • Time off the slopes:  6  weeks.
  • Time off the kite board: 8 weeks.
  • Time off house work:  forever!   Just kidding, usually 3 – 4 weeks.
  • How long does it last:  It depends on how what type of procedure was done.  Fat transfer sometimes needs to be repeated.  A buttock lift usually is long lasting unless there is a significant change in weight.
  • Patient satisfaction:   high

The fine print:

There are several procedures that I offer to treat the buttock area.

Sometimes liposuction of the hips and the lateral thighs can make the buttock look more shapely just by changing the proportions of the posterior body. It’s always important to look at the whole shape, not just one area.

Sometimes the buttock itself is disproportionately small. In those cases, fat grafting is an option.  The fat is harvested from areas with excess, purified and then carefully injected into the buttock. The injections are done into the subcutaneous fat layer which is very safe. I do not do transfer huge amounts of fat into the gluteus muscles.  I am aware that this operation is in demand but given the risk of the fat getting into the blood stream and causing severe problems, it is one that I do not do.  If you are looking for a gigantic booty, I’m not your surgeon.

As of July, 2018 the American Society of Plastic Surgeons (ASPS) and the American Society for Aesthetic Plastic Surgery (ASAPS) have issued an advisory against any injection into the gluteus muscles.  The risk of death is just too high (1 in 3000 as of this update).  I have been a member of both of these societies for many, many years and I cannot remember receiving such a strongly worded advisory.  This is serious stuff.  My concern now is that many surgeons doing this procedure are not board certified plastic surgeons and not members of these two organizations and that they will not get this advisory and will continue to inject into the gluteus.  I have seen some such Instagram posts that just make my head spin and my butt ache.  So ladies, and a few gents out there, that Kim Kardashian back side is not worth dying for.  Don’t go there!

Sometimes additional fat is not necessary.  In older patients or in those who have lost a lot of weight, the buttocks may become saggy and in those cases a buttock lift may be indicated. This is essentially a lower body lift that does not enlarge the buttock but raises it into a higher position.

Some doctors offer buttock implants made of solid silicone gel. I do not. I know that this can be a successful operation (very popular in Brazil) but I am concerned about putting large implant into the gluteus muscle and having a patient sit on that implant.  Again, if you a looking for a gigantic booty, I’m not your surgeon.
